If you would like to apply to be enrolled in Synergy Learning Center, please use the form below to Pre-Register and gain access to the Application Packet.  Fill out the forms and mail or fax them back to the information provided below. Once we receive your filled out application, we will contact you roughly two weeks prior to class for an intake evaluation.

Qualifying Criteria

  • Current mailing address and phone number
  • Current and/or prior work history
  • High School diploma or GED equivalent
  • Secondary education reading level
  • Basic understanding of computer skills
  • Econmic status at or below poverty level
  • Unemployed or near it
  • Ability to participate in the mentoring program.


*Criminal background checks will be conducted as part of the application process.
